Question B = Enter 1-6
Question C = Enter 1-4
Question D = Enter 1-4 Enter 5 for U
Question E = Enter 1-3 Enter 4 for U
Question F = Enter 1 if circled, 0 if not circled (per choice)
Question G-A = Enter last two digits of the year born*
Question G-B = Enter 1 for male, 2 for female
Question G-C = Enter 1 if circled, 0 if not circled (per choice)
Question G-D = Enter years lived in local areas*
Question G-E = Enter number of people in household*
Question G-F = Enter 1-4
Question G-G = Enter 1-7
Question G-H = Enter 1-4
