* Note: This survey is designed to print using modern browser technology. Those using Netscape 4.x or Internet Explorer 5.x or earlier browsers will find their printouts might not look correct, please upgrade your browser in order to print out the correct layout.

Internet Explorer 6.x users might wish to modify their page setup as follows:

1. Click "File", then "Page Setup". Clear the Header and Footer areas, and set the page margins all to 0.5. Click OK.
2. Click "Tools", then "Internet Options", then click the "Advanced" tab at the top of the window, and scroll down until you see the printer icon on the left and the heading "Printing". Check the box that says "Print background colors and images". You might want to go back and uncheck this after printing the survey.

Netscape 6.x users will find that there is no Page Setup option. There is no way to clear header and footer information, so you must print out the survey as is and white-out the header and footer information before you make copies, or use Internet Explorer.
